As I begin my 20th year in the field of dentistry, I am always encouraged that most people will keep their teeth for the rest of their lives. This wasn’t always the case, but thanks to modern dentistry and full mouth rehabilitation this dream is now a reality for many people.

Some patients are lucky enough to not need any rehabilitation. However, I see many patients where rehabilitation of worn teeth is essential to keeping all of their teeth for the rest of their lives.

My practice philosophy is “function over aesthetics”. By addressing the function we will accomplish a beautiful smile.

Most cases that I address involve orthodontics with Invisalign followed by restoring the teeth to the original shape with crowns to complete the rehabilitation. The need for the restoration is based on the delicate anatomy of a tooth.

The outer part of the tooth is made of enamel. It is one of the hardest substances in the body and is vital to the long term survival of a tooth. The second layer is dentin. It is about 10 times softer than enamel and wears much faster than the enamel. When teeth wear down, it causes more biting force on the other teeth which results in the wearing down of those teeth. 10 to 20 years of this pattern of tooth wear will then lead to tooth loss and fracture. Therefore, it is very important to address the issue of worn teeth.

Immediate loading reduces the need for surgical intervention and complex bone grafting procedures that might otherwise be required to restore resorbed ridges. The one-stage technique offers a simplified surgical workflow and subjects the patient to one surgery instead of two. Improvements in patients’ post-operative experience following flapless approaches for single-tooth rehabilitation have been reported. Successful cases of the immediate loading protocol have also been reported for multiple implant placement.
